At Lentor Senior Care Centre (LSCC), we draw inspiration from the symbolism of a well-rooted tree: the Tree of Compassionate Roots.

It guides us in our mission to provide person-centred care, foster a sense of inclusion, promote independence, and ensure the overall well-being of our seniors. Who are We?

We are the Team of 3-Ds: Dynamic, Determined and Dedicated team of skilled professionals and well-trained care assistants. Our goal is to create a warm and nurturing environment that will empower and enrich the lives of seniors through the services that we offer.

Our services include but not limited to Maintenance Day Care, Dementia Day Care, Community Rehabilitation and Centre-based Nursing.

What services do we provide?

Maintenance Day Care (MDC)

Maintenance day care service provide a fun and safe environment for our seniors during daytime where they can socialize, exercise and engage in various activities. It aims to prevent social isolation, cognitive and functional deterioration and serve as a respite for the caregivers.

Dementia Day Care (DDC)

Dementia day care service is a specialised program that provides care, supervision and support for individuals with dementia during the day. It aims to provide a dementia-friendly environment where our seniors can have a supportive and inclusive space that promotes safety, reduce anxiety, and enhance their overall well-being.

Community Rehabilitation (CR)

The aim of you or your loved ones to participate in the Community rehabilitation is to improve physical health, enhance mental well- being, promote social engagement, increase independence, prevent functional decline and support a healthy lifestyle.

Centre-based Nursing (CBN)

This service, rendered by SNB-registered nurse, provides specialized nursing interventions to our seniors enrolled in the centre service and community seniors residing within the centre vicinity. This will provide more convenience to our seniors as they don’t have to book appointments and go to polyclinic and hospitals for these special medical needs.

Each service charge depends on the level of government subsidy that senior is eligible to. You may contact us at 64516300 for the corresponding charges for each services.

To determine if you qualify for government subsidy and receive advice on subsidy application, it’s recommended to consult with a referring Medical Social Worker at a public hospital or polyclinic.
The Medical Social Worker will conduct a means-test using the National Means Testing System provided by MOH and will provide you with further information regarding your eligibility for a subsidy with our nursing home service.

We aim to foster an inclusive and familiar environment for your loved ones. Please contact the respective centers for arrangements.

Please note that there will be additional charges if your helper have her meals at the center or uses the same transport to get to the center.

A recliner deck chair or similar can be provided for resting or taking a one-hour nap (subject to availability) for our seniors to recharge before engaging in afternoon activities.

Meal provided includes:

  • Morning snack (e.g. sandwiches, porridge)
  • Lunch (e.g. rice with assorted dishes)
  • Afternoon Tea (e.g. local desserts, pastry, bread)

We commit to adhere to each individual client’s required diets and food preferences (e.g. soft, blended food).

We can arrange transportation services to and from specific locations within a 5km from Senior Care Centers subject to availability. Additional charges will apply. Caregiver transport and escort service is subject to availability.

The main goal of DDC is to manage behavioural and psychological symptoms of dementia and to prevent or stop the progression of the senior’s condition through the following:

The goal is to enable our clients to age in place with dignity, maintaining physical abilities and mental capabilities as much as possible.

We have a strong team of Allied Health staff who play a crucial role in Community rehabilitation. This team comprise Therapy Aides, Physiotherapists and Occupational Therapists who are registered with the Allied Health Professions Council (AHPC).
